I am Qualified, professional and caring Disability Support Worker with a passion for improving quality of life for the disadvantaged. I have experienced in various aspects of disabled care provision, from general domestic duties to the implementation of comprehensive care plans. I seek a role with a quality service provider where I can continue to help people, reach their goals and live full life. I have also got experience as a teacher at primary, junior and senior high school in Ghana for over 12years. I have worked on many different projects and employers across the world. Work responsibilities: • I have worked with various care set up such as – autistic children, elderly care, learning disability people, Mental illness / complex care needs, personality disorder and histories of offending. • Provide standard personal care including bathing, dressing and grooming • Serve and feed meals according to the diet plan assigned to the patients • Administer medicines and transport the patients to examination room for check up and treatment • Ensure that the patients can freely pursue their hobby and engage in recreational activities with other patients • Maintain confidentiality and prepare reports on individual behaviours and preferences • As a Learning Facilitator, I teach and assist permanent teachers to plan lessons for the both early childhood and disability children. Works alongside the clinician[s] in emergency management of patients and the provision of patient care I also undertake and record the following tasks: • Measurement of height and weight • Urinalysis • Measurement of blood pressure, temperature and pulse • Recording of ECGs, stool chart, fluid chart and food chart • Recognises changes in the patients’ recordings which must be reported and acted on. • Responsible for accessing and filing of patient notes • Clearly presents patients’ point of view to others • Advice on smoking cessation I also perform the following practice management duties such as: . Maintains set standards of organisation in treatment rooms . Takes appropriate action if standards are not being met . Ensures stock items are ordered and provided in GP & treatment rooms . Maintains clinical equipment efficiently and ensure servicing is undertaken . Follows infection control standards . Undertakes computer input data eg. Vaccinations, blood tests, observations . Works effectively in a team . Interacts with patients’ and colleagues in a professional manner . Worked for the individual growth and development of patients in the premises . Provided personal care assistance to the patients . Performed all other duties as assigned
